St. James Mercy Hospital’s Case Management department offers free discharge planning and social work services to patients and their families.
Our Case Managers help individuals with many different social, psychological or environmental needs that arise due to a patient’s illness, diagnosis or treatment.
As licensed registered nurses, our Case Managers understand a patient’s many needs, and act as a liaison between the hospital, patient, family, and community agencies. They also serve as a resource to other hospital departments and physicians to assist with long-term care placement, home care, insurance, medical equipment, rehab services, hospice, financial and protective issues, and more.
